My Life A Full Life

My Life a Full Life is the Island's opportunity to radically change the way health and social care is delivered to people on the Isle of Wight. My Life a Full Life has been bringing together organisations to make changes, empower people and communities to develop a sustainable health and social care system which enhances the Island population's health and wellbeing. The changes are to ensure communities are ready and resilient for changing times; people are encouraged to promote their own health and wellbeing; those who are most vulnerable receive the right kind of care and support; more care and support is available locally making the most of the full range of services, voluntary groups and independent providers in the community.

The programme is now powered by Vanguard. Vanguard is an NHS led initiative designed to support and help a number of areas to develop new models of care. In March, the Isle of Wight, along with 269 other health and social care partnerships, expressed an interest in leading the development of a new care model via My Life a Full Life and One Wight Health, with the aim of transforming how we encourage healthy living and how care and support is delivered locally. Following a rigorous process the Island was successful in becoming a New Models of Care Vanguard site. This means the Island will take a national lead in transforming care for people, families and carers and those using voluntary, private, health and social care services.
